Integrated Outreach Meeting
The purpose of this survey is to gather information from members of the Integrated Outreach Meeting (IOM) to help improve the efficacy of the meeting, reinforce what is working well, identify the types of services being offered in the community, and discover ways to better coordinate those services to support people experiencing homelessness. There are two tools being used to gather information: a survey and a map. Please complete both using the tabs below. The results of the survey will be anonymized. Please share the link to this survey with other coworkers who are outreach or frontline staff.

Service Provision Map
Please indicate where and what type of services you provide within the City of Abbotsford using the colour coded markers (no comments are necessary; click "submit" once the marker is placed; markers cannot be deleted once submitted but can be moved). You can place multiple markers and multiple types of markers to reflect the different services and locations you work. The 5 markers are as follows:
- Survival Supplies (tents, sleeping bags, blankets, clothing, water, food, etc)
- Harm Reduction Services
- Health Outreach (wound care, health checks, etc. provided outside of a medical facility such as outdoors or in a shelter)
- Case Management Outreach (supporting clients with their personal goals such as acquiring ID & MSDPR services, applying for housing, accessing wraparound service, etc.)
- Shelter Services (serving clients in a shelter as a shelter staff member)
